Police say a Nissan Xterra, driven by James Allen Jeffers, 33, of South Beloit, was traveling westbound on the bridge, crossed the median, and crashed head-on into a Chevy Malibu, driven by Alex Banks, 25, of Rockford. A passenger was also in the Malibu, Tanisha Gates, 23, of Rockford.
“This was a horrible, tragic event. Three children are now without a mother due to this defendant’s terrible decision to get behind the wheel while intoxicated,” said State’s Attorney Gibbons. “My heart goes out to these children and the family and friends of Samantha Miller who are mourning her death at such a young age.”
Fuentes-Vazquez was taken into custody for a Failure to Appear warrant for driving under the influence of an alcoholic beverage and with a valid driver’s license.

“Miguel Agustin-Tellez was killed because Mr. Luellen, drunk and high on a cocktail of drugs and alcohol, chose to flaunt the law. Taking drugs and then driving creates is a danger that far too often ends tragically,” Kane County State’s Attorney Joe McMahon said.
