On Saturday, May 27, 2017, at 2:34 AM, Troopers arrested Astrid Ortiz, age 25, of 5 Whelan Road, Providence, Rhode Island for 1.) Driving Under the Influence of Intoxicating Liquor and/or Drugs – 1st Offense/B.A.C. Unknown and 2.) Refusal to Submit to a Chemical Test.
The investigation showed that Mihali was traveling south in the northbound lanes of Route 146 as he entered Rhode Island from Massachusetts, Moynihan said. While traveling in the wrong direction in the high-speed lane, his car, a 1987 Cadillac, collided head-on with a Honda Accord approximately 800 feet south of the state line.

Salgado’s five and three year old sons were in the rear seat and were improperly restrained. Salgado failed a series of Standardized Field Sobriety Tests at the scene. She was placed under arrest and transported to the State Police Lincoln Woods Barracks where she refused to submit to a Breath Test.
At this time the crash is still under investigation but speed and alcohol are believed to be a factor in this crash. The operator Michael Woloshchuk was issued a citation for DUI refusal, LSA and Gross Negligent Operation before he was transported to Dartmouth Hitchcock Medical Center in NH.
